Senior Transitions Program
Senior Transitions is an inpatient, behavioral health program for seniors with emotional or behavioral issues requiring short-term acute stabilization. The 15-bed specialty unit located in Low Moor serves individuals who are referred to the program for the following conditions:
- Depression with feelings of hopelessness or suicide; inability to care for oneself
- Anxiety causing constant fears, paralyzing worries or panic attacks
- Severe disorientation or memory impairment
- Inability to maintain adequate nutrition which threatens life or vital body functions
- Threatening thoughts or assaultive behaviors towards others
- Unusual of bizarre behavior with illogical thoughts or beliefs
- Emotional issues with co-occurring substance abuse or medical issues
- Inability to maintain psychiatric stabilization through outpatient treatment
Admissions Criteria
Our program serves patients ages 65 and over who require acute inpatient treatment for one or more of the conditions listed above. Patients between the ages of 55 and 64 with mental health disabilities requiring acute stabilization will also be considered for admission provided they are appropriate for the treatment setting and would benefit from a program structured for the senior population.

Ask your physician for a referral or contact us directly at 540-862-6950, Monday through Friday, 8 a.m. to 5 p.m. We also accept walk-in patients during these hours.
